Camp Forms

Printable applications to CJL's regular summer session and to Parent-Daughter weekend are available here (when appropriate), as are the staff applications.

These are in Adobe's Portable Document Format (PDF), so you'll need Acrobat Reader or another PDF viewer to see and print them. Each form is 90k or (much) smaller in size.

Download the form you want, print it out, and mail it with your deposit to camp at the appropriate address.
